چکیده

Subthreshold stimuli may produce ventricular excitation when they fall within a limited period of the cardiac cycle. This phenomenon, known as the supernormal phase of conduction (Adrian, 1921), was shown in the anaesthetized animal to be from the beginning of the descending limb of the T wave, and the beginning of the U wave (Hoff and Nahum, 1938). Its presence in the human heart was first reported by Lewis and Master (1924). Wenckebach and Winterberg (1927) later doubted its existence, but several authors have since confirmed its presence (Scherf and Schott, 1939; Kline, Conn, and Rosenbaum, 1939; Pick, Langendorf, and Katz, 1962; Soloff and Fewell, 1960; Linenthal and Zoll, 1962; Burchell, Connolly, and Ellis, 1964; Linenthal and Zoll, 1965). This period of hyperexcitability of the heart has been invoked to explain the genesis of a number of ventricular dysrhythmias, including those extrasystoles occurring early in diastole immediately after the T wave (Mack, Langendorf, and Katz, 1947; Soloff and Fewell, 1960; Burchell, 1963). In this paper, we show the supernormal phase of conduction of the human heart by observation on the effects of subthreshold artificial stimuli from failed implanted pacemakers. The duration of this phase is determined and its relation to ventricular extrasystoles established.

The supernormal phase of intraventricular conduction.

The supernormal phase of conduction is a short period of paradoxically improved conduction which may occur during an early phase of the recovery cycle of cardiac conducting tissue. The situation is paradoxical, for the improved conduction occurs during a short critical early period, while later impulses are blocked or have greater conduction delays. The term is, in a sense, a misnomer since con...

Interrelationship between automaticity and conduction in Purkinje fibers.

Microelectrode studies of transmembrane potentials of canine Purkinje fibers show that phase-4 depolarization causes voltage-dependent changes in conduction and responsiveness similar to those occurring during repolarization at comparable levels of potential. Abnormalities ranged from simple slowing of conduction to decrement, unidirectional and bidirectional block, and unexcitability. Reentran...

Clinical and Experimental Evidence of Supernormal Excitability and Conduction

True supernormality of excitability and conduction has been demonstrated in normal Purkinje fibers in in vitro studies. In the clinical setting, supernormality of conduction is manifested better than expected. This phenomenon is much more common than previously thought, particularly in the presence of certain clinical conditions. If a careful scanning of the cardiac cycle is performed on all pa...

Supernormality in Bachmann's bundle. An in vitro and in vivo study in the dog.

A supernormal phase of conduction between right and left atria was observed in the exposed hearts of anesthetized dogs. Interatrial conduction time of premature atrial responses was reduced by as much as 17% during the early phase of diastole, relative to the conduction interval of basic driven responses. The supernormal phase lasted from 60 to 140 msec and was greater at slower basic driving f...
